Terms and Conditions 

 

 

Thank you for your interest in the services at Sarah Nicole Holistic LLC. I am here to provide you with Wellness Coaching guidance to support and improve your overall health and well-being. 

 

Prior to Sarah Nicole Holistic LLC agreeing to accept you as a client and provide 

you with wellness coaching services, the company requires that you 

review the terms and conditions explained below. Please note that prior to starting our work together you will be asked to sign a copy of the terms and conditions.  

 

Throughout this letter agreement the terms “client” shall refer to you the 

individual. 

 

Retaining Sarah Nicole Holistic LLC to provide  wellness coaching guidance to support and improve the clients health and well-being. Please understand that prior to Sarah Nicole Holistic LLC providing any services, the company will require client consent and the following representations, warranties and covenants: 

 

Understanding that a Wellness Coach does not diagnose or treat any 

disease, disorder or condition and is not a licensed naturopathic doctor, physician or dietician.  

 

The information collected in the client intake form regarding health history, dietary habits and lifestyle will be used to get an overall picture the client’s health so Sarah Nicole Holistic LLC can (i) safely recommend changes to improve the client's general health and well-being, (ii) educate the client about the benefits of sound nutritional choices and is not intended for use to prevent, diagnose or treat any existing condition or disease. 

 

The client will represent and warrant that all medical information provided by the client to Sarah Nicole Holistic LLC during the initial health assessment is true and correct. The client will advise Sarah Nicole Holistic LLC as soon as possible of any errors in the medical information provided to Sarah Nicole Holistic LLC or any changes to current health status, including any disease or ailments that the client may be suffering from, any medication or over the counter drugs that the client is consuming, allergies and whether the client is pregnant, suspect they may be pregnant or is breast-feeding. 

 

Should Sarah Nicole Holistic LLC suspect the client may have a disease, condition, or 

disorder, the client will be informed of this suspicion and advised to consult a physician or 

naturopathic doctor for a proper diagnosis. By sharing this suspicion, the client understands it is 

not a diagnosis or conclusion about the state of the client’s health. 

 

The client acknowledges and agrees that Sarah Nicole Holistic LLC has not made any warranties or representations of any kind regarding results that may be achieved from 

the services provided. The client understands that any results or outcomes arising from these services are individual and may vary from person to person. The results of the services 

provided are not guaranteed and do not expect the Wellness Coach will be able to anticipate and explain all risks and complications. With this knowledge, the client voluntarily consents to the suggested services provided by Sarah Nicole Holistic LLC.  

 

The client acknowledges and agrees that the services may lead to certain unforeseen complications, acknowledging that the client has the right to accept or reject any services suggested of their own free will and choice. 

 

The client recognizes that specific foods, herbs and supplements may create allergic and possible serious reactions, in particular, products containing nuts. The client acknowledges that these allergies may cause serious bodily harm. The client represents and warrants that they have provided Sarah Nicole Holistic LLC, during the initial health and assessment a list of all items which, to the best of my knowledge, I am allergic or sensitive to. 

 

The client understands that their identity will be protected at all times and, if necessary, identifying information will be altered to protect my privacy. The client understands that a record will be kept of the services provided to them. This record will be kept confidential and will not be released to others unless so directed by the client or unless law requires it. Notwithstanding, the client understands that Sarah Nicole Holistic LLC is legally obligated to supersede confidentiality if they become aware of current child abuse or neglect, threats or harm to any individual and/or serious threat of suicide involved with my case. The client understands that they may look at their records at any time and can request a copy of it by paying the appropriate fees. The client understands that information from their records may be analyzed for research purposes and that their identity will be protected and kept confidential. 

 

In consideration for Sarah Nicole Holistic LLC agreeing to provide the client with the 

agreed upon services, the client irrevocably and unconditionally releases and forever discharges Sarah Nicole Holistic LLC, its directors, officers, employees, agents and contractors and Sarah Nicole Holistic LLC successors, assigns, heirs and legal representatives (all such persons and entities being called the “Released Parties”) of and from all manner of actions, causes of action, suits, demands, debts, accounts, covenants, contracts, damages and all other claims whatsoever, which the undersigned or its successors or assigns ever had, now has or may in the future have against any of the Released Parties for or by reason of any cause, matter or thing related to or arising from the services provided to me including (but not limited to) any bodily harm or injury sustained by the client or any other person. The client acknowledges and agrees that the Released Parties will in no way be responsible for any bodily harm or injury suffered by the client as a result of the services. 

 

The client agrees and acknowledges that Sarah Nicole Holistic has the right to 

cease providing the services to the client at any time effective immediately, without any 

compensation to the client whatsoever. This letter of agreement will cover the entire course of  

services and the client is free to withdraw consent and to discontinue participation of those 

services at any time. 

 

Please note that due to the nature of the services provided, no refunds will be warranted.
